FORMS
Below are frequently used forms to request academic record information, adjustments to degree program, changes to personal information, or other helpful resources.
- FOR STUDENTS

PDF forms that require faculty permission: you may print the form, fill out by hand, and email a high-quality photo to faculty or give it to them in person; or, download the form, fill out electronically, save as PDF, and email directly to faculty. Faculty will return the form to the Registrar’s Office when they have approved it.
PDF forms that do not require faculty permission: you may print the form, fill out by hand, and email a high-quality photo to registrar@hope.edu or bring it to the Registrar’s Office in person; or, download the form, fill out electronically, save as PDF, and email to registrar@hope.edu.
